Travel between multiple RAP sites may be required including Repat, Flinders & Noarlunga hospitals.

Lead the difference for Patients across Southern Adelaide
This part-time, ongoing Principal Social Worker role at Southern Adelaide Local Health Network puts you at the forefront of Disability and NDIS practice across three of Adelaide's major hospital sites at Flinders Medical Centre, Repatriation Health Precinct and Noarlunga Hospital. You'll bring advanced clinical expertise to complex cases while guiding and developing the broader social work team's NDIS capability. If you're an experienced Social Worker ready to lead meaningful change for patients navigating disability support within a hospital setting, this is a role with real scope and purpose.
About the role:
Reporting to the Director, this role is responsible for ensuring that long stay complex patients of the Division of Rehabilitation, Aged Care and Palliative Care (RAP) are managed in a timely and effective manner
Duties for the role will include:
Providing consultation, supervision, performance monitoring and professional development for social work staff,
Monitoring workloads and assisting with social work service provision during periods of high demand,
Developing and maintaining systems for early identification of complex potentially long stay patients in the Division of RAP (particular focus on < 65 year olds),
Identifying barriers to discharge from hospital and working to resolve these barriers in partnership with staff both internal and external to the hospital,
Maintaining Social Work processes to ensure they are consistent with current hospital and community demand/processes and professional practice guidelines
About you:
A degree in Social Work that entitles the individual to membership of the Australian Association of Social Workers,
Demonstrated ability to provide supervision and training to other social work staff,
Demonstrated ability to negotiate and develop a range of partnerships, networks and relationships with key stakeholders within and external to the organisation,
Demonstrated ability to think and work strategically, proactively and independently to develop creative and effective solutions to problems and manage change,
Demonstrated high level of oral and written communications skills and the ability to develop reports, proposals and presentations,
Extensive experience working as a Social Worker within a Hospital Setting,
Extensive experience in providing clinical management to complex and difficult cases and transitions,
Extensive experience working with the National Disability Insurance Agency
