Join our Support at Home team across Ryde, Wahroonga, the Northern Beaches and surrounding areas, supporting older adults to stay safe, independent and doing the things they love, in the place they call home. This is a fully mobile role allowing you to take ownership of your day with opportunities to work from home or in our beautiful clinic spaces for your reporting, documentation and admin.

What You'll Be Doing:
The Support at Home Team supports our geriatric clients in the community through the government-funded Support at Home program.
You'll have the opportunity to:
- Complete comprehensive functional assessments in clients' homes
- Identify barriers to independence, safety and participation
- Recommend and coordinate appropriate home modifications and equipment
- Develop practical strategies that support clients to remain independent
- Work alongside clients, families, carers and support coordinators
- Collaborate with other Allied Health professionals to achieve the best outcomes
- Advocate for your clients and ensure their goals remain at the centre of their care
- Build meaningful relationships with older adults and their support networks
- Keep documentation clear, accurate and clinically relevant
- Continue developing your skills through mentoring, clinical supervision and professional development

The Essentials You'll need:
- Degree in Occupational Therapy (or equivalent)
- Current AHPRA registration
- Current Australian Driver's Licence and access to a reliable vehicle
- NDIS Worker Screening Check
- Working with Children Check
- Willingness to travel across Ryde, Wahroonga, the Northern Beaches and surrounding areas
