Manager Chronic Care & Health Promotion (New Position)

Full-time, 38 hours a week

T his is an Aboriginal Designated Position, classified under 'special measures' of Section 12 of the Equal Opportunity Act 2010. Only Aboriginal and Torres Strait Islander people are eligible to apply.

About the Role

The Manager, Chronic Care and Health Promotion provides strategic and operational leadership across chronic care and health promotion services, ensuring culturally safe, accessible and holistic services for Aboriginal people, families and the community. The role leads the development and delivery of chronic care and prevention initiatives, supports the Care Coordinator to provide effective client care coordination and follow-up, and develops a comprehensive Health Promotion Plan based on community needs and health priorities. The Manager is also responsible for workforce leadership, financial management, reporting, partnerships and continuous service improvement.

About You

To be successful for this position, you should possess the following qualifications and qualities:

You are Aboriginal and/or Torres Strait Islander.
Have a relevant tertiary qualification in Management or willing to obtain.
Demonstrated understanding of Aboriginal health, culture, community and the social determinants that influence Aboriginal health outcomes.
Demonstrated experience developing and delivering culturally appropriate health promotion, prevention and community education programs.
Demonstrated experience in workforce management, staff supervision and performance development.
Demonstrated experience using data and community feedback to identify service needs and improve health outcome.
