Proactive Care is seeking experienced General Practitioners to deliver chronic disease management and structured multidisciplinary care for veterans and patients with complex, multi-system health needs. The role blends a roaming clinic component — seeing patients across a range of clinic and community locations in your metro area — with remote telehealth case-conferencing sessions.
This is not a standard locum, and it's a long way from ten-minute, high-volume telehealth. You'll provide clinical leadership within integrated Chronic Care Teams, working alongside allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful and genuinely multidisciplinary — a model that values thorough care over high-volume, transactional throughput.
We work across both Medicare and DVA billing streams, with a dedicated focus on veteran health and chronic disease management.
Remuneration
You start on a guaranteed hourly rate while you build your patient base, then move to a 70% share of billings once you're established. That gives you a secure floor from day one, with room to earn more as your list grows.
First 3 months (guaranteed): $240.00 per hour
After 3 months: 70% of billings

Location & working options
This is a metro-based role combining travel across your city with remote telehealth days.

Roaming clinic days: you'll travel across your metro area on a semi-frequent basis, seeing patients at a range of partner clinic and community locations rather than being tied to one site.

Remote telehealth days: structured case-conferencing sessions delivered from your home office (a suitable space and reliable internet are all that's required).
Available across Adelaide, Sydney, Melbourne, Brisbane and Perth. Working days are structured each week and arranged around you, confirmed by location at application.
The Role
As a General Practitioner within our Chronic Care Team model, you will provide clinical leadership across in-community consultations and structured telehealth case conferencing:

Conduct comprehensive GP consultations for acute, chronic and complex conditions across a range of clinic and community locations

Deliver preventative health assessments — cardiovascular risk, metabolic health, mental-health screening and lifestyle risk modification

Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)

Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams, integrating clinical, pathology, imaging and allied-health input into coordinated care plans

Initiate, review and manage pathology and imaging investigations; provide prescribing and medication management; and refer to allied health, specialists and community services

Ensure clinical safety, prioritisation and escalation pathways are clearly identified and actioned
Working pattern: the role is built around structured days each week — roaming clinic days across your metro area and remote telehealth case-conferencing days (approximately 22 patient reviews over an 8-hour session, conducted via telehealth). There is flexibility on which days these fall, arranged around your availability.
Health & Focus
Proactive Care has a dedicated focus on veteran health and chronic disease management for both Medicare and DVA-funded patients. You'll work with patients who often present with complex, multi-system conditions — chronic pain, mental-health comorbidities, musculoskeletal injuries.
Requirements
Essential

Medical degree (MBBS or equivalent) and current registration with the Medical Board of Australia (AHPRA)

Fellowship with RACGP or ACRRM (VR GP), or eligibility for a 19AB exemption in an eligible DPA location

Experience in chronic disease management, preventative health and complex care coordination

Competence in prescribing, referrals and investigation management

Strong communication and collaboration skills within multidisciplinary teams

Willingness to travel across metro clinic and community locations, and a suitable home-office setup for telehealth days
Desirable

Experience with telehealth consultations and digital-health platforms

Experience within team-based or multidisciplinary care models

Interest or experience in veteran health, DVA processes or the CVC Program

Experience with GP Management Plans (GPMPs), Team Care Arrangements (TCAs) and chronic disease care planning
