Purpose of Position
The Care Manager is responsible for the training, development, and compliance of carers, scheduling and monitoring operational performance and performing nursing care to Client/Participants in accordance with an established plan of care in adherence with Right at Home policies and procedures and all other laws and relevant professional standards.
The Care Manager (Registered Nurse, Enrolled Nurse, Physiotherapist or Occupational Therapist) will oversee the care and coordination of all allocated Client/Participant related activities within the Right At Home service, ensuring that a high quality of consumer-directed care is delivered to all Client/Participants within the statutory requirements.
In addition to Care Management, this role may contribute to developing relationships with hospitals, discharge planners, clinics, and care facilities with a view to delivering care services to meet the market. The Care Manager also manages the care coordination (care partnering) team.
Client/Participant Focus
· Dealing with a range of clients, participants, and their families daily, this role will handle anything from information requests, feedback, general enquiries, to complaints.
· Serves as a liaison between referral sources, families, carers, and administrative staff members.
· Use a variety of superior interpersonal communication skills to deliver genuinely respectful Client/Participant engagement, including active listening, gaining all customer details, information gathering, problem analysis and solving, persuasiveness, and negotiation.
· Provides Client/Participant and representative education on the nature of services that are available and the effect and benefit of the care plan.
· Maintain oversight and review clinical in-home care assessments and care plans to best meet Client/Participant requirements.
· Performs the duties of a nurse or allied health professional as needed (if within scope).
· Monitors the clinical quality of Client/Participant care, including compliance with all Right at Home care and clinical policies and procedures.
· Supports and implements improvements in line with the Aged Care Standards and other relevant legislation. Ensures training of care staff on clinical competencies, administration and arranges education as required.
Administrative Functions
· Responsible for the intake and risk management of client needs.
· Ensures compliance with all relevant legislation.
· Ensures proper billing in line with the program guidelines, and direct care is captured correctly.
· Compliance with WHS regulations.
· Maintain oversight of Right at Home Incident Management, as well as reducing risk by monitoring clinical indicators and client outcomes.
· Attend all mandatory Home Office lead training and meetings and share learnings and teaching flows through to the appropriate team members.
Business Functions
· May assist management with business development opportunities across a range of referral sources.
· May represent the Company at conferences, health fairs and other community service functions, and participates in civic and/or community committees as requested.
· May actively network with relevant industry bodies & professional contacts.
· Commit to a focus on continuous improvement of care processes and procedures.
· Maintain oversight of clinical and care staff, including regular performance reviews.
Documentation
· Complete documentation promptly and accurately in Right at Home systems, following organisational policies and legislative requirements.
· Maintain comprehensive records and documentation in accordance with quality standards, frameworks and organisational policies and procedures.
Leadership
· Undertake all duties within existing qualifications, scope of practice, skills, knowledge, and experience, identifying areas for personal development and improvement.
· Participate in ensuring exemplary quality processes are adhered to and maintained, and ideas and innovations are encouraged and explored.
· Oversee and maintain knowledge of the clinical governance framework, including but not limited to high risk, high prevalence and open disclosure.

Qualifications
Educational Qualifications
· Current AHPRA Registration as a Registered Nurse, Enrolled Nurse, Physiotherapist or Occupational Therapist – Essential)
· Relevant Health Science degree – Essential.
· Relevant tertiary postgraduate qualification (Aged Care, Geriatric, Management) – Desirable.
· Certificate in a specific nursing-related discipline (Dementia care, geriatric nursing, palliative care) – Desirable.
· Postgraduate studies in business, management, and/or finance – Desirable.
Experience
· Minimum of three years of experience in a similar role.
· Knowledge of the aged care quality standards and other relevant legislative requirements – Essential.
· Previous experience in Aged Care or related discipline – Desirable.
Skills, Knowledge and Abilities
· Fulfils the obligations of Nurse or Health Professional, including the Code of Ethics, Right at Home Code of Conduct and the Aged Care Code of Conduct.

Key Selection Criteria
· Current AHPRA registration (Registered Nurse, Enrolled Nurse, Physiotherapist or Occupational Therapist).
· Appropriate qualifications and a demonstrated commitment to the Aged Care Sector and the Home Care environment.
· Australian Federal Police Clearance Certificate.
· Driver’s License and a comprehensively insured, dependable private vehicle.
· Ability to assess, plan, implement, and evaluate in-home care and services. Knowledge of the Aged Care & Community Care Sector, Continuous Quality Improvement, and applicable Legislation.
· Working knowledge of My Aged Care and Services Australia
· Demonstrated ability to lead and actively contribute to organisational effectiveness.
· Demonstrated excellence in interpersonal/communication skills (written and oral), and an ability to lead and motivate the Care Team in the achievement of organisational goals.
· Understanding scope of practice and delegation of roles and tasks as required.
· Demonstrated ability to assist in the provision of holistic Client/Participant centered care using a systematic approach
· Demonstrated ability to communicate effectively with Client/Participant, representatives and other members of the Care Team.
· Ability to maintain safety for clients, self, and others, with knowledge of emergency procedures and WHS legislation.
· Demonstrated knowledge of the Legislation applicable to the Role of the Care Manager in the Community Care.
· Demonstrated understanding of open disclosure, risk management and quality principles.
· Demonstrated evidence of a commitment to ‘ongoing learning’ and updating professional skills and knowledge by attending courses with CDP credits.
In-Service Training
- Participates in mandatory education and ongoing In-Service training (This requirement need not be met exclusively through Right at Home-sponsored in-services programs).
Working Conditions/Environment
- Primarily works in clients’ place of residence and office. Shifts or visit times and days vary with clients’ needs.
