About Us
Nepean Blue Mountains Local Health District (NBMLHD) is an exceptional environment to advance your career while enhancing your skills and expertise. The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Join our dedicated Community Health team based at Lithgow Community Health Centre for this full time opportunity. As a Registered Nurse, you will provide high-quality, person-centred nursing care to individuals, families and carers across a variety of community settings, including clients' homes and community clinics.
Working within a multidisciplinary team, you will deliver a broad range of services including wound management, palliative care, chronic disease management and continence care. The successful incumbent will be responsible for assessing, planning, implementing and evaluating care using evidence-based practice and a primary health care approach, while partnering closely with clients, carers, GPs and other healthcare providers.
This role offers the opportunity to make a meaningful difference in the community by supporting clients to maintain their health, independence and wellbeing. You will also provide supervision and support to Enrolled Nurses, ensuring safe and effective delegation of clinical care. This position is based on a 7-day rotating roster and uniform is required.
