Step into a dynamic and rewarding opportunity as a Clinical Nurse Specialist – Transfer of Care Coordinator, where you’ll play a pivotal role in ensuring timely, and well‑planned patient discharges.

Drawing on your recent acute care nursing experience, you’ll work across the DMU, transit lounge, and inpatient wards to identify potential discharges, coordinate optimal patient care, and escalate any barriers such as radiology or patient transport delays. This seven‑day service offers structured hours—Monday to Friday from 06:30 to 15:00, and Saturday to Sunday from 09:00 to 17:30—providing a balanced and engaging role for a nurse ready to make a meaningful impact on patient flow and care continuity.

Experienced and motivated clinician ready to take on a new challenge and contribute

Attention to detail with a strong focus on accuracy, safety, and high‑quality patient care
Active listening to ensure clear communication, patient understanding, and effective coordination of care
Good communication in particular with the multi-disciplinary teams (e.g. – pharmacy; radiology; allied health and medical officers)
Sound clinical judgement with the identity potential discharges
Be autonomous in your practice
Timely escalation
Show initiative with the discharge transfer process
Identify quality improvement strategies to meet the hospital needs

Selection Criteria

Do you currently hold Authority to Practice as a Registered Nurse with AHPRA, relevant post-registration qualification and at least 3 years full time equivalent experience working the clinical area of your qualification?
Give a brief overview of what education training and clinically relevant experience you bring with you to the role? (limit to 2-3 lines)
Efficient patient flow is essential for hospital capacity, how can you positively influence patient flow in the hospital, tell us about your experience and some examples of how you have achieved this?
What makes you as suitable candidate for the position? What experience do you have in the acute care setting.
Can you give me some examples of how your clinical expertise and leadership can be used to influence timely patient flow discharge within the wards/ hospital.
