Opportunity 1 – Outpatient Pharmacist - 12 Month Maximum Term Contract
Reporting to the Pharmacy Outpatient Services Manager, the role focuses on delivering high-quality outpatient pharmacy services for patients of Lifehouse. This role supports timely access to cancer medicines, patient counselling, dispensing, treatment verification and safe medication use across outpatient and ambulatory care settings. The successful candidate will work closely with pharmacists, technicians, nurses, medical staff and patients to support safe, efficient and patient-centred pharmaceutical care.
Grade 1 Pharmacists with community pharmacy experience are encouraged to apply.
- Commitment to delivering high-quality, patient-centred pharmaceutical care and ongoing professional development.
- Provide outpatient dispensing, medication counselling and patient education for cancer and supportive care medicines.
- Support safe access to medicines through outpatient pharmacy workflows, including prescriptions, supply coordination and medication information.
- Collaborate with multidisciplinary teams to support continuity of care between inpatient, outpatient and community settings.
- Maintain accurate dispensing, clinical and administrative records in accordance with organisational and legislative requirements.
- Participate in pharmacy service improvement, medication safety activities and team-based workflow optimisation.
Opportunity 2 – Rotational Clinical Pharmacist - 12 Month Maximum Term Contract
Reporting to the Pharmacy Inpatients Manager the role provides clinical and operational pharmacy services across cancer care, perioperative and acute-care settings. The role will rotate through key clinical areas and contribute to safe medication use, patient education, dispensing services, technician supervision and quality improvement activities.
- Provide clinical pharmacy service across medical oncology, surgical oncology, perioperative medicine assessment, intensive care and palliative care.
- Complete best possible medication histories and clinical medication reviews for acute cancer patients.
- Support safe use of systemic anti-cancer therapy, oral chemotherapy, immunotherapy and supportive care medicines.
- Review medication orders, post-operative medicines, including analgesia, antimicrobials, VTE prophylaxis and discharge prescriptions.
- Support dispensing, clinical verification including chemotherapy, controlled drug governance and compliance with legislative requirements.
- Organise and supervise technician workflow, including preparation, labelling and supply of medicines.
- Participate in ongoing quality improvement practice in medication safety and translational research studies in cancer care.
- Provide medication education and advice to patients, carers, pharmacy staff, students, interns and health care professionals.
- Liaise and collaborate effectively with other healthcare professionals to support safe and timely patient care.

Essential Requirements
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Bachelor of Pharmacy or equivalent
- Completion of the eviQ Pharmacy Anti-cancer Drug Course or willingness to complete as required.
- Relevant hospital, outpatient or community pharmacy experience suitable to the position applied for.
- Up-to-date drug knowledge, with interest or experience in oncology, cancer care or supportive care medicines desirable.
- Excellent written and verbal communication skills to effectively interact with members of the health care team, patients, carers, contractors and vendors.
- Strong knowledge of medication safety principles, including medication reconciliation, transitions of care and discharge planning.
- Ability to prioritise workload across inpatient, outpatient, perioperative and acute-care settings.
- An understanding of the delivery of cancer care in a tertiary institution or willingness to develop this knowledge.
- Legal right to work in Australia
- Current Working with Children Check (WWCC)
- National Police Check (within the last 6 months or willingness to obtain)
- Compliance with Category A immunisation requirements
