About the Role
We are seeking an experienced and passionate Intensive Case Manager to join the Forensic Mental Health Service at Ravenhall Correctional Centre.
As an Intensive Case Manager, you will coordinate care for people with significant mental illness within a custodial environment. You will undertake comprehensive mental health assessments, develop and review recovery plans, coordinate multidisciplinary interventions and advocate for positive consumer outcomes.
Working within a collaborative team, you will engage with internal and external stakeholders to support recovery-focused care, risk management and continuity of treatment for consumers.
Key Responsibilities
- Coordinate clinical care for an allocated caseload.
- Conduct comprehensive mental health assessments and reviews.
- Develop, implement and evaluate Mental Health Recovery Plans.
- Collaborate with multidisciplinary teams and correctional stakeholders.
- Participate in clinical meetings, case reviews and service improvement activities.
- Monitor and manage clinical risk.
- Maintain high-quality clinical documentation and reporting.
- Support a recovery-oriented and trauma-informed approach to care.
About You
You will be a skilled mental health clinician who is committed to delivering high-quality, person-centred care in a complex environment.
Essential Requirements
- Current registration with AHPRA as a Registered Nurse.
- Postgraduate Diploma in Mental Health Nursing (or equivalent).
- Extensive experience in mental health nursing.
- Strong assessment, care planning and case management skills.
- Excellent written and verbal communication skills.
- Demonstrated knowledge of contemporary mental health practice and relevant legislation.
- Ability to work effectively in a complex multidisciplinary environment.
Desirable
- Experience in forensic mental health, correctional health or other secure settings.
