About Our Partner – Interactive Community Care (ICCare)
Interactive Community Care (ICCare) was established in 2010 as a specialist provider in the Community Services Sector and has proudly supported people across NSW for more than a decade.
As a multidisciplinary community care organisation, ICCare delivers out-of-home care, crisis support, and disability services across the Hunter, Central Coast, Sydney, Mid-North Coast, and Western NSW regions. ICCare is committed to meeting the complex care needs of children, adolescents, and adults.
About the Role
Lumiere Foundation is seeking an experienced NDIS Support Coordinator to work with ICCare participants in greater Sydney.
In this role, you will provide varying levels of Support Coordination and Transition of Supports, assisting participants to implement their NDIS plans, connect with services, and achieve their goals while exercising choice and control.
In This Role, You Will

Apply and adhere to NDIS and Child Safe standards and regulations
- Efficiently coordinate supports for participants within allocated funding levels
- Liaise and collaborate with participants, families, service providers, and other stakeholders to achieve participant goals
- Develop and maintain effective service linkages and community connections
- Empower participants to exercise choice and control through formal, informal, mainstream, and community-based supports
- Identify opportunities for innovative and person-centred service delivery
- Maintain accurate records, case notes, files, and reports in line with NDIS requirements
- Ensure compliance with NDIS Quality and Safeguards standards and legislative requirements
What You’ll Bring

Tertiary qualification in Social Sciences, Health, Community Services, or a related field, or a Diploma in Disability Services
- Minimum 2 years' experience in NDIS Support Coordination
- Strong understanding of the NDIS Quality and Safeguards Commission Practice Standards
- Experience working within the disability and community services sectors
- Proficiency in Microsoft Office Suite (Outlook, Word, Excel)
- Experience using client management systems such as MYP, Lumary, ShiftCare, or similar
- Excellent verbal and written communication skills
- Strong report writing and documentation skills
- Ability to speak Arabic is highly regarded but not essential
