About the Role:
- Support compliance with the Aged Care Act 2024, Strengthened Aged Care Quality Standards, Support at Home, CHSP requirements, NSW Public Health directives, and organisational policies and procedures.
- Deliver evidence-based nursing care, including health monitoring, wound management, medication support, chronic disease management, and clinical interventions within scope of practice and within the scope of registration with the Nursing and Midwifery Board of Australia (NMBA) and Australian Health Agency Practitioner Regulation Agency (AHPRA)
- Promote dignity, choice, diversity, inclusion, and culturally safe practices in all aspects of service delivery.
- Work as part of a team to ensure clients are supported in their independence, physical, psychosocial, social and spiritual wellbeing in accordance with their care plan
- Facilitate improvements to service quality and responsiveness inclusive of assessment/s and development
- Contribute to the performance of Council: demonstrate professional conduct, make the best use of knowledge, experience and skills and be accountable for own decisions and actions
- Contribute to a safe and respectful environment for consumers, the workforce and visiting practitioners that supports and encourages positive partnerships, and encourages consumers and the workforce to identify and raise concerns about clinical quality and safety
About You:
To be successful in this role you will have:
- Current Nursing and Midwifery Board of Australia (NMBA) and Australian Health Practitioner Regulation Agency (AHPRA) registration
- Demonstrated experience in clinical assessment, care planning, case management, and the delivery of person-centred care
- Sound knowledge of contemporary aged care practices, including the Strengthened Aged Care Quality Standards, Support at Home Program, CHSP, and relevant legislation
- Well-developed communication and interpersonal skills, with the ability to build effective relationships with clients, families, carers, multidisciplinary teams, and external stakeholders
- Demonstrated ability to work autonomously, manage competing priorities, and exercise professional judgement in a community setting
- Strong organisational, administrative, and time-management skills
- Demonstrated understanding of client rights, dignity of risk, duty of care, and safeguarding responsibilities
- Proficient in the use of computers
- Commitment to work at times out of hours if the demand exists including occasional weekends and public holidays
- Current Working With Children’s Check
- Current Australian Police Check
- Current Class C Drivers Licence
- First Aid
Desirable criteria:
- Minimum 3 years' post-registration nursing experience, preferably within community, aged care, home care, primary health, or chronic disease management settings
- Experience working within a local government, community services, or not-for-profit environment
- Experience supervising, mentoring, or providing clinical guidance to support staff
- Experience participating in accreditation, quality audits, and continuous improvement initiatives
- Knowledge and understanding of the Aged Care Act 2024, Strengthen Aged Care Quality Standards, the Support at Home Program and the Commonwealth Home Support Program (CHSP)
