About the Role
AB Nurses Support Services is seeking an experienced and compassionate Care Coordinator to join our growing NDIS team.
The Care Coordinator will be responsible for coordinating participant supports, communicating with participants and their families, liaising with support workers and external providers, and ensuring services are delivered safely, consistently and in line with participant goals and NDIS requirements.
This is a client-focused role requiring excellent communication, organisation and problem-solving skills.
Key Responsibilities
- Coordinate day-to-day supports for NDIS participants.
- Develop and maintain strong relationships with participants and families.
- Coordinate support workers and ensure shifts are appropriately covered.
- Prepare and manage staff rosters according to participant needs.
- Communicate with support workers regarding shifts, availability and participant requirements.
- Monitor service delivery and participant satisfaction.
- Liaise with families, Support Coordinators, Plan Managers, allied health professionals and other service providers.
- Follow up participant concerns, incidents and service issues.
- Maintain accurate participant and service records.
- Ensure required documentation and progress notes are completed.
- Assist with onboarding and orientation of support workers.
- Monitor staff compliance, qualifications, training and required checks.
- Support participants with changes to their services and care requirements.
- Escalate risks, incidents and safeguarding concerns to management.
- Ensure services are delivered in accordance with NDIS policies and organisational procedures.
- Assist management with general care coordination and operational duties.
Essential Requirements
- Previous experience in NDIS, disability, aged care, community services or healthcare.
- Experience in care coordination or service coordination is highly desirable.
- Strong understanding of NDIS participant supports and service delivery.
- Excellent communication and interpersonal skills.
- Strong organisational and time-management skills.
- Ability to manage multiple participants and priorities.
- Confident communicating with participants, families and support workers.
- Professional telephone and email communication.
- Strong computer and administrative skills.
- Current NDIS Worker Screening Check and Working with Children Check, or willingness to obtain.
- Ability to work independently and as part of a team.
Sponsorship Opportunity
Potential sponsorship may be available for a suitable and highly experienced candidate, subject to applicable Australian visa, sponsorship and employment requirements.
Sponsorship will be considered based on the candidate's qualifications, skills, relevant experience, performance and business requirements. Sponsorship is not guaranteed.
3-Month Trial/Probation
A 3-month trial/probation period will apply to assess performance, reliability, communication, participant management, coordination skills and suitability for the role.
