Service Coordinator – Job Description

Position Summary
The Service Coordinator is responsible for coordinating participant services and support workers to ensure safe, consistent and person-centred support in line with participant needs and NDIS requirements.
Key Responsibilities
- Coordinate participant services, support workers and staff rosters.
- Communicate with participants, families, guardians and other stakeholders.
- Monitor service delivery, staff attendance and participant needs.
- Maintain accurate participant records, agreements and service documentation.
- Coordinate shift replacements and address staffing issues.
- Follow up on incidents, complaints and participant concerns.
- Ensure staff qualifications, screening and training requirements are maintained.
- Liaise with Support Coordinators, Allied Health professionals and other service providers.
- Support NDIS compliance, audits and continuous improvement.
Requirements
- Experience in NDIS, disability, aged care or community services.
- Strong communication, organisation and problem-solving skills.
- Good understanding of person-centred support.
- Good computer and administration skills.
- NDIS Worker Screening Check and other relevant checks.
- Certificate III/IV in Individual Support, Disability, Community Services or related field is desirable.

Pay: $45.00 – $55.00 per hour
Expected hours: 8.0 per week
Education:
- Bachelor Degree (Required)
Licence/Certification:
- AHPRA registration (Required)
- Nursing Qualification (Preferred)
