We are excited to be expanding our NDIS team and are seeking two experienced NDIS Support Coordinators / Recovery Coaches to join our team at our Head Office in Burswood, WA.
These are full-time positions supporting NDIS participants across the Perth region. You will work closely with participants, families, support networks and service providers to develop personalised support plans, coordinate services, manage risks and help participants achieve their goals.
Experienced Support Coordinators are also welcome to bring their existing participant caseload with them when joining Roshana. This provides an opportunity to continue supporting your existing participants while benefiting from the resources, services and broader supports of an established organisation.
The successful candidates will also have the opportunity to build strong relationships across the Perth community and contribute to the continued growth of Roshana’s NDIS services.
- Uphold and promote Roshana Care Group’s values and principles in all interactions and decision-making.
- Maintain professional, ethical and respectful relationships with participants, families, colleagues and external stakeholders.
- Build strong relationships with mainstream, NDIS-registered and non-registered service providers.
- Connect participants with appropriate community, clinical and support services.
- Collaborate with multidisciplinary teams to achieve positive, best-practice outcomes.
- Develop and maintain professional networks across the Perth community.
- Identify opportunities to strengthen Roshana’s NDIS services and participant portfolio.
- Support the continued growth of Roshana’s NDIS services across the Perth region.
- Work collaboratively with participants and families to identify goals, strengths and support needs.
- Promote independence, recovery, social connection and the development of life skills.
- Provide individualised, trauma-informed, person-centred and family-inclusive support.
- Promote holistic wellbeing, including physical and mental health.
- Attend NDIS plan review meetings and assist participants to prepare.
- Liaise regularly with Support Coordinators, Plan Managers, NDIA staff and LACs.
- Develop, implement and regularly review participant support plans and risk assessments.
- Provide psychosocial recovery coaching.
- Develop service agreements in line with participant plans and available funding.
- Maintain a safe and healthy work environment in accordance with all relevant WHS requirements.
- Take responsibility for your own safety and the wellbeing of others.
- Report hazards, unsafe practices or equipment.
- Use personal protective equipment as required.
- Address and report breaches of health and safety policies appropriately.
- Minimum Certificate IV in Mental Health, Disability or a related/equivalent qualification.
- Valid NDIS Screening Check.
- Valid Working With Children Check.
- Valid Australian Driver's License.
- Current First Aid and CPR Certificates.
- Strong understanding of person-centred practice and professional boundaries.
- Ability to work effectively within a multidisciplinary team.
- Excellent written and verbal communication skills.
- Strong commitment to participant wellbeing, dignity and choice.
- Experience navigating NDIS plans and funding arrangements.
- Previous experience in Support Coordination, Recovery Coaching or a related role.
- An established participant caseload and professional network within the Perth region.
- Knowledge of contemporary NDIS support provisions.
- Experience using client management systems.
- Sound IT skills and confidence using standard business applications.
