Salary: $116,892 - $120,680 + super + salary packaging
New FY26/27 wage increase to be announced soon. All approved increases will be backdated to 1 July once implemented.

About the Role:
- Max-term role working with Aged Care inpatients at St Vincent’s Hospital. Delivering advanced social work interventions as a key member of a dynamic multidisciplinary team.
- Participate in the 24-hour on-call social work crisis service, delivering timely support and interventions outside standard hours.
- Provide comprehensive psychosocial social work services within Aged Care, delivering timely support to patients, clients, families and carers while actively contributing to interprofessional clinical practice.
- Lead care coordination and discharge planning by collaborating with hospital teams, community services, and external agencies to ensure safe patient transitions, including arranging accommodation and ongoing support.
- Advocate for patients and carers both inside and outside the hospital as appropriate, including where required referral for My Aged Care and application to the NDIS, NCAT: Guardianship division.
- Provide clinical supervision, education, and consultation to staff and students, and contribute to quality improvement, research, and professional development aligned with St Vincent’s Hospital’s mission and policies.
About You:
- Qualified Social Worker with a degree in Social Work, eligible for membership with the Australian Association of Social Workers (AASW) with extensive clinical experience in health settings, including advanced skills in aged care social work across hospital and/or community environments.
- Strong understanding and experience in managing complex social issues faced by patients, families and carers within aged care, including navigating aged care systems, carer stress, and psychosocial impacts of ageing and chronic illness.
- Demonstrated advanced clinical skills and knowledge in psychosocial assessment, counselling, case management, advocacy and complex discharge planning.
- Recognised clinical leader providing expert social work advice in multidisciplinary teams, skilled in managing high-demand critical care environments, promoting consumer participation in decision-making, and leading service improvements to enhance patient care and outcomes.
- Excellent interpersonal, communication, negotiation, and organisational skills, with a commitment to collaborative team practice, professional development, and mentoring junior staff and students through supervision and education.
