About the Role
Churches of Christ Queensland are currently seeking an experienced Care Manager to join our Palms Aged Care Service, a 50-bed residential aged care service located in the stunning location of Ingham, North Queensland.
This is a key clinical leadership role responsible for supporting the delivery of safe, high-quality and person-centred care to our residents. Working closely with the Service Manager, you will provide clinical leadership, guidance and support to our nursing and care teams while driving quality, compliance and positive resident outcomes.
As a Care Manager, you will play an important role in creating a culture where residents receive compassionate, individualised care and employees are supported to deliver their best.
Key Responsibilities

Lead, support and develop Registered Nurses, care staff, allied health professionals and volunteers.

Oversee resident assessments, care planning and clinical documentation.

Monitor clinical practice, care outcomes and quality indicators.

Coordinate care delivered by internal teams and external healthcare providers.

Review AN-ACC information to support accurate and appropriate submissions.

Ensure clinical programs, policies and procedures meet legislative, regulatory and professional requirements.

Support incident management, risk mitigation and continuous improvement.

Build strong relationships with residents, families, employees and healthcare professionals.

Promote a collaborative, accountable and resident-focused team culture.
This position is designated as a Responsible Person under the Aged Care Act 2024 and plays an important role in supporting compliance with the Aged Care Quality Standards, Code of Conduct and reportable incident requirements.
About You
The ideal candidate will be an experienced nursing professional who is passionate about clinical leadership, quality care and making a difference in the lives of older people.
Essential:

Current registration with AHPRA as a Registered Nurse.

Minimum five (5) years' post-graduate experience in aged care or an acute clinical setting.

Demonstrated experience in clinical leadership and people management.

Strong knowledge of contemporary aged care practice, clinical governance and quality standards.

Well-developed communication, leadership, decision-making and problem-solving skills.

Confidence managing clinical risk, change and continuous improvement.

Current CPR certification and unrestricted driver's licence.

Current National Police Certificate, or ability to obtain.

Valid Australian working rights - unfortunately, we are not currently able to sponsor.
Desirable:

Postgraduate qualification in gerontology, dementia care or another aged care-related discipline.

Experience with AN-ACC and aged care funding processes.
