Senior Clinical Pharmacist Mental Health Drug and Alcohol

About the role
As an experienced mental health pharmacist, you will lead the development and implementation of a community mental health team (CMHT) pharmacist position in WNSWLHD. You will join a supportive pharmacy department who are committed to excellence in patient care and who seek opportunities to develop practices which enhance and improve the patient experience.
Your leadership and clinical pharmacy skills will be utilised to improve the care of our MHDA consumers in the community by providing essential services within the CMHT, including, but not limited to:
- Medication reconciliation at admission and discharge
- Involvement in multidisciplinary team meetings, including engagement with Consultant Psychiatrists and/or Psychiatry trainees in medication decision making
- Monitoring psychotropic medicines, including side effect and therapeutic drug monitoring (e.g. clozapine, lithium)
- Metabolic and cardiometabolic risk monitoring and management
- Education provision to consumer, carers and staff
- Liaison with GPs and community providers for continuity of care
Experience with caring for MHDA clients is essential.
