Join South Western Sydney Local Health District's Acute and Extended Care Community Nursing teams and make a meaningful difference by providing safe, high-quality, person-centred care to patients in their own homes and community clinics.As a Registered Nurse, you will care for patients with a range of complex acute, chronic and palliative care needs, working collaboratively with patients, families, carers and the multidisciplinary team to achieve positive health outcomes.This role provides an opportunity to support hospital avoidance, facilitate timely discharge and enable patients to receive quality care closer to home, promoting independence and continuity of care.You will have the opportunity to broaden your clinical knowledge and develop your community nursing skills across a diverse range of clinical presentations, with support from experienced nursing and multidisciplinary teams.

We are looking for a compassionate and motivated Registered Nurse who is committed to delivering safe, high-quality and person-centred care and is passionate about making a difference in the community.The successful applicant will demonstrate:Strong clinical assessment, decision-making and critical thinking skills.Ability to work independently and confidently across home-visiting and community clinic environments.Excellent communication and interpersonal skills, with the ability to build positive relationships with patients, families, carers and colleagues.Ability to work effectively as part of a multidisciplinary team.Flexibility, adaptability and resilience when responding to varied and changing patient needs.Sound clinical judgement and a strong commitment to patient safety and positive health outcomes.Ability to provide evidence-based nursing care and manage patients with complex health needs.Commitment to professional development, accountability and continuous improvement.Experience or demonstrated skills in areas such as Hospital in the Home, wound care, PIVC/CVAD, surgical drains, catheter care and palliative care will be highly regarded. An interest and willingness to further develop skills in community nursing is also valued.
