Care Coordinator – Aged Care & Disability
Central Coast / Newcastle | Work From Home | Part-Time or School Hours | $40 per hour
Are you organised, calm under pressure and great with people? HomeCare Australia is looking for a capable Care Coordinator to support our Care Partners, clients and families across aged care and NDIS services. This is a practical, hands-on coordination role for someone who enjoys keeping things moving, following up details and making sure clients feel heard and supported.
A genuinely flexible role: work from home, part-time or school hours, at $40 per hour.
What you’ll be doing
· Be a first point of contact for clients and families, returning routine calls and helping resolve day-to-day queries.
· Pre-screen new client enquiries and pass relevant information to the Case Management team.
· Prepare service agreements and budgets for aged care and NDIS clients, and keep client paperwork current and accurate.
· Gather and document information about health changes, mobility, continence, cognition and wellbeing using structured questioning.
· Maintain accurate client notes, tasks and communications in Visual Care.
· Coordinate appointments, service confirmations and follow-up documentation such as GP summaries and discharge letters.
· Track unresolved issues, complete delegated follow-up tasks and provide concise daily updates to Care Partners.
· Identify and promptly escalate clinical risks, incidents, deterioration or matters outside your scope to the Care Partner or RN.
· Liaise professionally with families, support workers, GPs, allied health providers, contractors and internal teams.
· Participate in daily run-sheet meetings and help maintain continuity of care during busy periods or staff leave.
This role will suit you if you are
· A confident communicator who enjoys speaking with older people, participants and families.
· Highly organised and detail-focused, with strong follow-through and the ability to keep multiple tasks moving.
· Calm, practical and solution-focused when priorities change.
· Comfortable working independently from home while staying closely connected with a supportive team.
· Experienced in aged care, disability, health administration, community services or a similar client coordination environment.
· Confident using cloud-based systems, Microsoft Office and/or Google Workspace.

Mandatory requirements
· Current Driver’s Licence.
· National Police Check.
· NDIS Worker Screening Check, or willingness to obtain one.
· Reliable NBN plus backup internet and a dedicated home office environment.
