What you'll be doing
To provide expert clinical leadership and coordination of comprehensive Palliative and End of Life Care Services across a defined geographical area.
The CNS2 – Coordinator is responsible for the daily triage, allocation, intake, and clinical oversight of palliative care patients, supporting multidisciplinary service delivery, ensuring timely access to high quality, evidence-based care, and promoting clinical mentorship, continuous service improvement, and person-centred practice.
About You
You are a dedicated and highly skilled Registered Nurse with current registration through the Nursing and Midwifery Board of Australia and a strong foundation in specialist palliative care. Supported by a relevant postgraduate qualification and extensive recent clinical experience, you deliver compassionate, evidence-based care to patients with complex and evolving needs.
Your advanced clinical assessment skills enable you to confidently manage challenging presentations, utilising Palliative Care Outcome Collaboration (PCOC) assessments to inform clinical decision-making, prioritisation of referrals, triage processes, and comprehensive care planning. You excel in coordinating care across diverse settings, effectively managing caseloads while ensuring patients and families receive timely, high-quality support throughout their palliative care journey.
As an experienced clinician and leader, you are passionate about developing the capability of others through education, mentorship, and clinical guidance within multidisciplinary teams. You foster collaborative working relationships and are respected for your ability to communicate effectively with healthcare professionals, patients, families, and carers, ensuring person-centred care remains at the forefront of service delivery.
Driven by a commitment to excellence, you actively engage in professional development, quality improvement initiatives, and the meaningful use of clinical data to enhance service outcomes and patient experiences. Flexible and responsive to service needs, you hold an unrestricted NSW Driver's Licence and are able to travel across the Mid North Coast Local Health District to support the delivery of specialist palliative care services.
