About the Role
Are you an experienced Mental Health Social Worker looking to step into a senior clinical role where you can make a meaningful difference for people experiencing acute mental health challenges? Join the Agile Psychological Medicine (aPM) Clinic as a Grade 4 Social Worker (Senior Clinician) and provide specialist psychosocial intervention to consumers, families and carers as part of a highly skilled multidisciplinary team.
People accessing aPM often present during periods of crisis, where mental health challenges intersect with complex psychosocial factors. Your role will be to help address these barriers so consumers can engage meaningfully in psychological treatment and focus on their recovery and wellbeing.
Working from Casey Hospital, in this role, you will:
Provide advanced psychosocial assessment and Social Work interventions to consumers, families and carers
Work collaboratively with consumers to identify psychosocial needs and develop recovery-focused plans
Address factors impacting recovery, including housing, family violence, AOD, financial and broader social support needs
Partner with Psychologists, Psychiatrists and other clinicians to provide coordinated, evidence-based care
Work closely with internal and external services to support continuity of care and reduce unnecessary handoffs
Apply your senior clinical expertise to support high-quality, person-centred practice within the aPM team
About the Program
The Agile Psychological Medicine (aPM) Clinic provides timely, focused and evidence-based treatment for people accessing public mental health services.
Consumers are referred from services including Psychiatric Triage, Acute Crisis Intervention Service (ACIS) and other areas across the Mental Health and Wellbeing Program. Many are experiencing a period of crisis alongside underlying mental health conditions and complex life stressors.
At the heart of aPM is a commitment to consumer-centred care, shared decision-making and continuity. The clinic aims to reduce the number of handoffs people experience when navigating a large public mental health system, providing clear clinical pathways alongside specialist psychological, medical and psychosocial care.
About You
You are an experienced and compassionate Mental Health Social Worker who brings strong clinical judgement, advanced psychosocial skills and a genuine commitment to recovery-oriented care. You will bring:
Advanced psychosocial assessment and intervention skills
Experience supporting people experiencing complex mental health and psychosocial challenges
Confidence working with consumers, families and carers within a recovery-oriented framework
Strong collaboration skills and the ability to work effectively across multidisciplinary and external service networks
An adaptable approach and the ability to exercise autonomy and sound clinical judgement
A commitment to evidence-based, person-centred and family-inclusive practice
