About the Opportunity
The Hunter Valley Lower Mid North Coast (HVLMNC) Mental Health Service is a dynamic community mental health service comprising Care Coordination, Intake and Acute Care teams located across Taree, Forster, Gloucester, Bulahdelah and the Hunter Valley. This Clinical Nurse Specialist Grade 2 (CNS position is based at 560 High Street, Maitland, within our Hunter Valley Mental Health Service. We provide acute outpatient mental health services to adults and their families across the Hunter Valley and Lower Mid North Coast communities. Our service provides comprehensive consultation, assessment, treatment and follow-up, with a strong focus on supporting consumers as they transition from hospital to community-based care.
Our teams support consumers through medication monitoring, psychoeducation, relapse prevention, psychiatric review coordination and liaison with families, carers and community support services.
You will be encouraged to participate in planned in-service education, as well as virtual and face-to-face training opportunities relevant to your role and professional development.
What You Will Be Doing
As a CNS2 – Mental Health Clozapine Clinic, you will provide specialist nursing practice, complex clinical care, advanced clinical skills and leadership across Clozapine Clinic services.
Your responsibilities will include:

Coordinating the Clozapine Clinic within the assigned service.

Overseeing satellite Clozapine Clinic service provision.

Coordinating the care of consumers participating in the GP Shared Care Program.

Providing complex case management to Clozapine clients where required.

Providing specialist clinical consultation and advice across a mixed clinical environment.

Supporting safe and effective medication monitoring and management.

Providing education and training to consumers, families, carers and healthcare professionals.

Providing mentoring, coaching and supervision to Registered Nurses.

Supporting evidence-based practice and contemporary approaches to mental health care.

Working collaboratively with internal and external service providers to support continuity of care.

Contributing to quality improvement and service development initiatives.

Promoting person-centred care and positive health outcomes for consumers.
You will be directly supported by Senior Leaders and the Team Manager, including the HVLMNC Mental Health Service Manager, while working closely with the broader Mental Health team to deliver high-quality, contemporary care.
