The Housing Support for the Aged Program supports people 50 years and older with complex health needs and a history of homelessness. It helps them maintain long-term public housing and improves their health and wellbeing.
The Practitioner will develop collaborative work practices to assist clients to access a variety of services, will assist clients to set and achieve goals, and will advocate for clients where required.
In addition, the Practitioner of the Housing Support for the Aged program will be responsible for maintaining a manageable case load of clients of their own volition, managing the support journey of clients effectively, closing supports when all achievable goals have been reached.
