Registered Nurse (RN) – Support at Home (SAH)
We are seeking an experienced and compassionate Registered Nurse (RN) to join our Sydney team on a part-time, office-based basis, supporting Support at Home (SAH) participants with complex and high-level clinical care needs.
About the Role
As an RN, you will play a key clinical leadership role in coordinating and delivering high-quality, person-centred care. You will work closely with participants, families, support workers, medical practitioners and allied health professionals to ensure each participant’s individual needs, goals and care outcomes are effectively managed.
Key Responsibilities
- Conduct comprehensive clinical assessments and triage for SAH participants.
- Develop, implement and regularly review individualised care plans based on participants’ clinical needs and personal goals.
- Provide clinical leadership and support for participants with complex care requirements.
- Use strong clinical assessment and critical decision-making skills to identify and respond to changes in participants’ health and care needs.
- Coordinate and liaise with GPs, specialists, allied health professionals, families and other healthcare providers.
- Advocate for participants and promote their dignity, independence, wellbeing and quality of life.
- Provide clinical guidance, supervision and support to Personal Care Workers.
- Ensure care delivery is consistent with relevant legislation, standards, policies and best-practice principles.
- Support ongoing compliance with practice accreditation and quality requirements.
- Maintain accurate clinical documentation and participant records.
Skills & Clinical Experience
The successful candidate will have demonstrated experience in aged care, community care or complex clinical support, including:
- Participant triage and comprehensive clinical assessments.
- Development and review of individualised care plans.
- Catheter management and catheter changes.
- Enteral feeding, including PEG and PEJ management.
- Bowel and bladder management and relevant sign-offs.
- Manual handling and safe transfer practices.
- Seizure management.
- Wound assessment and management.
- Clinical risk assessment and escalation.
- Supporting quality, compliance and accreditation requirements.
Mandatory Requirements
- Current Registered Nurse (RN) registration with AHPRA.
- Current NDIS Worker Screening Check.
- Valid Working with Children Check (WWCC).
- Current CPR and First Aid certification.
- Current Australian driver’s licence and access to a reliable, roadworthy vehicle.
- Full Australian working rights.
- Excellent communication, interpersonal and teamwork skills.
- Ability to work independently, manage competing priorities and meet deadlines.
- Sound computer skills and confidence using electronic care management and documentation systems.

Pay: $55.00 – $70.00 per hour
