Child, Youth and Family Mental Health Service (CYF-MH) provide assessment and specialist community Mental Health services to children, young people and their families across the Nepean Blue Mountains Local Health District (NBMLHD). CYF-MH is committed to working in partnership with families and the systems around them to provide person centred, trauma informed and recovery orientated care.
Are you a passionate and experienced Clinical Nurse Specialist looking to make a meaningful impact in Child, Youth and Family Mental Health (CYF-MH)?
Join our dedicated multidisciplinary team in Penrith and play a pivotal role in supporting children, young people, and their families to access timely, coordinated, and person-centred Mental Health care. This is an exciting opportunity for a motivated Clinical Nurse Specialist to work within our Care Navigation service, helping consumers and families navigate the mental health system while ensuring seamless coordination across health, community, and social care services.
As a Clinical Nurse Specialist – CYF-MH Care Navigation, you will provide advanced clinical expertise, care coordination and leadership to support children, adolescents, and families experiencing Mental Health challenges. You will work collaboratively with internal and external stakeholders to facilitate access to appropriate services, improve continuity of care, and promote positive health outcomes.
This role will see you develop collaborative care plans that are recovery-oriented and family-centred, coordinate referrals and transitions between acute, community, primary care, education, and non-government services, provide clinical consultation and support to multidisciplinary team members and be an advocate for consumers and families to ensure equitable access to appropriate services.
