We are seeking a passionate and experienced Program Coordinator to join our newly funded Tokoko Integrated Recovery Pathways Program in the Wide Bay region.
This exciting role will support people transitioning into, through and beyond residential rehabilitation. Working closely with clients, families, community organisations and service providers, you will help strengthen recovery outcomes through transition planning, community reintegration, housing pathways and ongoing recovery support.
About the Role
As the Program Coordinator, you will play a key role in supporting people to successfully transition from residential rehabilitation into sustainable recovery within the community.
A core focus of the role is coordinating post-residential support and the program's transitional accommodation pathway, helping individuals build the skills, confidence and connections needed to maintain recovery and move towards independent living.
Working collaboratively with residential teams, external service providers and community stakeholders, you will help ensure people remain connected to the supports they need throughout their recovery journey.
Key Responsibilities
- Support clients through pre-admission, transition and post-residential recovery pathways.
- Coordinate access to transitional accommodation and support successful tenancy outcomes.
- Assist clients to build independent living skills, recovery supports and community connections.
- Develop and implement transition and recovery plans tailored to individual needs.
- Support clients to access housing, health, employment, education and community services.
- Strengthen continuity of care through effective planning, communication and service coordination.
- Build and maintain strong referral pathways and relationships with community stakeholders.
- Support relapse prevention and ongoing recovery planning.
- Advocate for clients and assist them to navigate complex service systems.
- Maintain accurate documentation, case notes and reporting requirements.
This role is based in Bundaberg and will involve regular travel throughout Bundaberg, Hervey Bay and the wider Wide Bay region.
What We're Looking For
To succeed in this role, you will bring:
- Qualifications in Community Services, Human Services, Social Work, Psychology, Counselling or a related discipline, and/or relevant experience.
- Experience supporting people through complex support pathways, transitions or recovery-focused programs.
- Experience in case management, care coordination, housing support, tenancy support or community services.
- Understanding of recovery-oriented, trauma-informed and person-centred practice.
- Knowledge of housing, homelessness, community and health service systems.
- Strong stakeholder engagement and relationship-building skills.
- Excellent communication, advocacy and interpersonal skills.
- Ability to work autonomously while contributing positively within a multidisciplinary team.
- Strong organisational and documentation skills.
- Current Australian Driver Licence.
Experience within residential rehabilitation, transitional accommodation, homelessness services, aftercare or post-treatment recovery services will be highly regarded.
