We are seeking a Care Coordinator (EEN or AIN), to join our team at the Sacred Heart Monastery Assisted Living facility on a part-time basis. This is a vital position within our senior care operations that sits at the heart of the care delivered. As the Care Coordinator, with the other nursing staff and carer, you will play a pivotal role in ensuring seamless coordination of care services, supporting both the Priests and Brothers. You will enjoy the role caring for well seniors, making this a highly rewarding and meaningful role.

Key responsibilities
- Coordinate and drive the men to appointments, ensuring best care is arranged, medications delivered as ordered and the care is documented.
- Communicate effectively with care staff, nurses, allied health professionals and external service providers to ensure quality care delivery.
- Assess residents' changing needs and coordinate appropriate responses, liaising with the Director of Care and RN, to adjust care provisions as required.
- Participate in handover meetings to ensure continuity of care.
- Identify and escalate any issues or safeguarding concerns to appropriate management

- Demonstrable experience working in an aged care or healthcare setting, has a drivers licence and able to drive and support seniors at appointments and follow-up on any care required.
- A caring person who enjoys providing best care in a faith based organisation.
- Excellent organisational and time management skills with the ability to manage multiple tasks and priorities.
- Outstanding communication skills, both written and verbal, with the ability to liaise effectively with diverse stakeholders.
- Proven ability to work collaboratively within a multidisciplinary team environment.
- Compassion, empathy and a genuine commitment to person-centered care and resident wellbeing
- Computer literacy and proficiency with electronic care management systems.
- Has current First Aid, CPR certification.
- Arrange outings e.g. movies, shopping.
Pay: $35.00 – $40.00 per hour
