About Hometown Nursing Care
Hometown Nursing Care is a registered NDIS provider based in Lakemba, supporting participants across Sydney to live independently and take part in their communities. We’re a close team, and the people we support stay with us — so we hire for reliability and genuine care, then back you with training and real support from your coordinator.
The role
We’re looking for Disability Support Workers to provide in-home and community-based supports to NDIS participants. Shifts are available across mornings, evenings, weekends and sleepovers, and we’ll build a roster that works around your availability.
What you’ll be doing
- Assisting participants with daily personal activities, including personal care, hygiene and mobility
- Supporting participants to take part in community and social activities
- Helping with household tasks, meal preparation and shopping
- Supporting travel and transport to appointments and activities
- Building life skills and independence in line with each participant’s NDIS goals
- Keeping accurate progress notes and reporting any changes or incidents promptly
What we’re looking for
- Genuine interest in supporting people with disability to live the life they choose
- Reliable, punctual and comfortable working independently in participants’ homes
- Clear communication and a respectful, person-centred approach
- Ability to follow support plans and document your work accurately
Essential requirements
- NDIS Worker Screening Check (or willingness to obtain before starting)
- Working with Children Check
- NDIS Worker Orientation Module certificate
- Current First Aid and CPR certificate
- Australian driver’s licence, a reliable vehicle and comprehensive insurance
- Full working rights in Australia
Well regarded but not essential
- Certificate III or IV in Individual Support, Disability or Community Services
- Manual handling and medication administration training
- Experience with complex care, behaviour support plans or high-intensity supports
- A second language — our participants and their families come from many backgrounds
