Purpose of Position
The Registered Nurse (Community) performs nursing care to clients/participants in accordance with an established plan of care in line with Right at Home policies and procedures and all other laws and relevant professional standards.
The Registered Nurse (Community) provides holistic client-focused and consumer-directed care in accordance with an established care plan. The Registered Nurse (Community) provides for the personal needs and comfort of clients in their own home, ensuring work within enablement and wellness principles to support the clients’ goals.
Duties
· Adheres to all Right at Home organisational policies and procedures.
· Participates in Continuous Improvement, including monitoring of clinical indicators and follows risk management open disclosure frameworks
· Promotes the professional development of less experienced staff through ongoing education.
· Supports and implements the Aged Care Quality Standards and observes and respects the client/participant’s rights, ensuring an environment which promotes safety, security, dignity, and optimal health for clients/participants in their own home.
· Knowledge of, and compliance with, all relevant Legislation, Regulatory requirements, Professional Standards and Guidelines that affect the work practices of the individual and those of subordinate staff.
· Ensures that documentation is timely & reflective of client/participant care, assessments, and evaluations. Participates in individualised education for clients/participants' clinical care needs to ensure safe delivery of care.
· Communicates and liaises effectively with key stakeholders, both internal and external, ensuring that the goals of the client/participant are met.
· Communicates effectively and maintains harmonious interpersonal relationships with clients/participants and other staff through written, verbal and non-verbal modes.
· Maintains privacy and confidentiality.
· Participates in annual performance appraisal to identify achievements and goals for ongoing professional development.
· Maintains annual CPD requirements as required by AHPRA.
· evidence-based in-home nursing care, including wound care, catheter care and insertion and medication administration.
· Ensure efficient daily workload planning, including travel, documentation, reporting, and administrative tasks
Pay: $57,828.39 – $110,483.71 per year
