As a Care Coordinator, you'll work closely with members, healthcare providers and multidisciplinary care teams to support individuals managing chronic health conditions or requiring hospital substitute treatment. You'll play a key role in coordinating care, providing education and support, and helping members navigate their healthcare journey to achieve better health outcomes.

What will you do?
- Identify and engage eligible members through referrals, enquiries and outreach programs.
- Support the assessment, development and monitoring of personalised care plans.
- Coordinate care with members, healthcare providers, service providers and broader care teams.
- Provide health education, guidance and support within your clinical scope of practice.
- Maintain accurate clinical records and contribute to quality care outcomes.
- Participate in clinical audits, professional development and continuous improvement initiatives.
- Contribute to a positive, safe and member-focused team culture.
What are we looking for?
- Tertiary qualification in Nursing, Allied Health or a related health discipline.
- Minimum two years' post-qualification clinical experience.
- Strong communication and interpersonal skills with a passion for helping people.
- Excellent organisation and time management skills.
- Ability to work collaboratively with members, clinicians and service providers.
- Sound computer skills, including Microsoft Office applications.
- Current registration with the relevant professional association.
- Experience in care coordination, chronic disease management or health coaching.
- Experience delivering services via telehealth.
- Strong customer service experience within a healthcare environment.
