About the opportunity
We are seeking an experienced Healthy Ageing Team Leader to provide operational leadership across our southern region programs, including Support at Home (SaH) and the Commonwealth Home Support Programme (CHSP). In this role, you will lead a multidisciplinary team of at least 6 direct reports, including Care Partners, Occupational Therapists, Exercise Physiologists, and Nurses, to deliver high-quality, person-centered care.
About you
You will bring at least 5 years’ experience leading teams within aged care or community services, and be confident:
- Overseeing day-to-day operations, service delivery targets, workforce utilization, and intake/assessment workflows for SaH and CHSP.
- Providing leadership, coaching, and supervision to a multidisciplinary care team.
- Maintaining compliance, quality improvement, and accurate reporting, including Data Exchange (DEX) requirements.
- Navigating aged care reform implementation, incident management, escalations, and budget/operational risks.
- Building strong relationships with consumers, carers, external referrers, and community partners.
What you will need
- Minimum 5 years leading teams within aged care or community services, with explicit experience in CHSP and Support at Home.
- Tertiary qualification in Aged Care, Community Services, Social Work, Business Management, Health, or a related field.
- Current QLD Class C Driver's Licence, NDIS Worker Screening Check (or Yellow/Blue Card).
- Deep understanding of Aged Care Quality Standards, funding legislation, and sector reform priorities.
- Strong operational capabilities (budget/data management, reporting), multidisciplinary team leadership, and stakeholder communication.
Desirable
- Experience in change management and organizational development.
- Knowledge of trauma-informed care and contemporary person-led support models.
