Position Summary
Correlate Connections is seeking an experienced, motivated, and compassionate Physiotherapist to join our growing multidisciplinary team as an independent subcontractor.
The successful contractor will provide comprehensive physiotherapy assessment, treatment, rehabilitation, education, and preventative services to clients across the lifespan. Working collaboratively with clients, families, carers, support workers, medical practitioners, and other allied health professionals, you will deliver evidence-based interventions that promote independence, improve mobility, reduce pain, prevent falls, and maximise participation in everyday life.
This role offers flexibility, autonomy, and the opportunity to work across a diverse caseload while being supported within a collaborative clinical environment.
Key Responsibilities
- Conduct comprehensive physiotherapy assessments for clients across the lifespan.
- Assess mobility, strength, balance, posture, gait, pain, functional capacity, and physical performance.
- Complete falls risk assessments and develop evidence-based falls prevention strategies.
- Develop individualised treatment and rehabilitation plans based on client goals and best practice.
- Deliver physiotherapy interventions including:
- Exercise prescription
- Strength and conditioning
- Balance and mobility training
- Gait re-education
- Manual therapy
- Pain management
- Neurological rehabilitation
- Musculoskeletal rehabilitation
- Functional rehabilitation
- Support clients recovering from injury, illness, surgery, or declining mobility.
- Provide physiotherapy services within homes, schools, workplaces, community settings, residential aged care facilities, retirement villages, and the clinic.
- Assess and recommend mobility aids, equipment, and environmental modifications in collaboration with other allied health professionals.
- Educate clients, families, carers, and support workers on safe mobility, exercise programs, injury prevention, and self-management strategies.
- Monitor client progress and modify treatment plans as required.
- Prepare comprehensive clinical reports for NDIS, Home Care Packages, CHSP, Residential Aged Care, Medicare, DVA, general practitioners, specialists, insurers, schools, employers, and other stakeholders.
- Maintain accurate, timely, and confidential clinical documentation.
- Participate in multidisciplinary case conferences and collaborative care planning.
- Maintain professional standards in accordance with AHPRA and the Australian Physiotherapy Association Code of Conduct.
- Participate in continuing professional development and clinical supervision where appropriate.
- Promote the values, professionalism, and reputation of Correlate Connections.

Essential Selection Criteria
Applicants must possess:
- Current registration as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA).
- Current Professional Indemnity Insurance.
- Current Public Liability Insurance.
- Australian Business Number (ABN).
- National Police Check.
- Working with Children Check (or relevant State/Territory equivalent).
- NDIS Worker Screening Check.
- Current First Aid and CPR certification.
- Current unrestricted Australian Driver Licence.
- Reliable motor vehicle with comprehensive insurance.
- Excellent written, verbal, and interpersonal communication skills.
- Strong organisational and time management skills.
- Demonstrated ability to work independently.
- Excellent clinical reasoning and evidence-based practice skills.
- Commitment to person-centred care and professional ethics.
Desirable Criteria
- Experience working with NDIS participants.
- Experience providing community-based physiotherapy services.
- Experience working within Support At Home (SAH).
- Experience in paediatric physiotherapy.
- Experience in neurological rehabilitation.
- Experience in musculoskeletal physiotherapy.
- Experience in falls prevention and balance rehabilitation.
- Experience with chronic disease management.
- Experience prescribing mobility equipment and assistive technology.
- Dry Needling, Clinical Pilates, Hydrotherapy, or other postgraduate qualifications.
- Experience working within multidisciplinary allied health teams.
- Knowledge of My Aged Care and reablement principles.
Contractor Responsibilities
The successful subcontractor will be responsible for:
- Maintaining all required professional registrations, licences, checks, and insurance.
- Managing taxation, superannuation, and financial obligations.
- Maintaining Continuing Professional Development (CPD) requirements.
- Providing invoices in accordance with the subcontractor agreement.
- Maintaining professional standards at all times.
- Complying with Correlate Connections' policies, procedures, privacy requirements, and quality standards.
- Maintaining clinical equipment as required.

The Physiotherapist will be expected to:

- Maintain compliance with AHPRA standards and Correlate Connections policies.
- Contribute positively to the multidisciplinary team.
- Build and maintain professional relationships with referral partners and stakeholders.
